Job Description

This is a 1 FTE Temporary Full time Position until approximately March 2027, starting immediately. The successful candidate will have a choice to continue as a casual pharmacist after the term ends.

Pharmacy Dept. plays a critical role in ensuring safe drug distribution and optimum clinical care to our patients in compliance with the legal, regulatory and accreditation standards. As a part of pharmacy team and a key member of multidisciplinary team, Pharmacist is responsible for the provision of pharmaceutical care and range of clinical services to our patients, and provide medication related consultations and education to the patients, staff and care providers. This position reports to the Director of Pharmacy and Quality Improvement.

Position responsibilities

Includes but not limited to:

  • Responsible for the provision of clinical pharmacy services with the purpose of optimizing outcomes to improve the patients’ quality of life.
  • Direct, patient-centered care in partnership with patients/family in a multi-disciplinary team environment to address drug therapy issues.
  • Perform medication and polypharmacy reviews, participate in quality improvement activities, adverse event reporting, provide drug information and promote evidence-based practices.
  • Order entry and verification using critical thinking and expertise to provide therapeutic check for safe drug distribution in Oracle Cerner Computer Provider Order Entry (CPOE) environment.
  • Other duties as assigned.
Job Requirements

Qualifications

  • Bachelor of Science in Pharmacy is required, Pharm D an asset.
  • Current registration with the Ontario College of Pharmacists required – Part A.
  • Previous hospital experience, preferred. Relevant advance certification or Master’s degree qualification will be an asset.
  • Demonstrated ability to work independently, and, as a member of a team.
  • Demonstrated ability to prioritize a consistent and varied workload.
  • Ability to direct and teach staff.
  • Demonstrated written and verbal communication skills required, in order to facilitate effective communication with patients, peers, other health care providers, and volunteers.
  • Demonstrated ability to identify, prevent and resolve drug related problems.
  • Good understanding of change management; must be able to identify change needs and proactively act on those needs.
  • Effective negotiation and conflict management/ resolution skills required. Excellent attendance and discipline free record required.
  • Working knowledge of use of computers and information technology (e.g. Windows Operating System, Microsoft Office), hospital/ Pharmacy information system and closed loop medication management systems is required.
  • Prior experience with Oracle Cerner, automated packager PACMED and PACVISION, Medidose, BD Pyxis automated dispensing cabinets is preferred.
  • Evidence of good performance history and attendance record.
  • Proficiency in both official languages (English and French) will be considered an asset.
