Responsible for the provision of Clinical Pharmacy services; resolving and preventing drug related problems through medication profile review, patient assessment, Pharmaceutical Care Plan Development and Implementation. Professional activities include provision of drug information to other health care providers including physicians, nursing, and other allied health professionals. Participating in committees, both internally and externally, conducting Drug Utilization Evaluation projects and delivering in services to professional staff.
Basic Education and Experience Requirements:
- Doctorate of Pharmacy degree (Pharm D) is preferred.
- Certification of competency from the Pharmacy Examination Board of Canada.
- Licensed as a Pharmacist with the Ontario College of Pharmacists.
- Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training and experience is preferred.
- Working experience in hospital setting with direct involvement in Pharmaceutical patient care (preferred).
- Strong knowledge in therapeutic drug monitoring.
- Good understanding of clinical pharmacist’s role within the multidisciplinary team.
- In-depth understanding of use, composition and clinical effects of drugs.
Responsibilities:
As a Clinical Pharmacist you will work to provide the following services to our patients:
- Provision of pharmaceutical care to optimize drug utilization, identifying potential and actual drug related problems, making recommendations and monitoring for efficacy and toxicity of drugs to achieve desired patient outcomes
- Provision of drug information, education to patients and all health care professionals
- Work in collaboration with physicians, and other health care providers to develop implement and monitor therapeutic plans
- Assist in the development of drug therapy guidelines for individual disease conditions
- Participate in medication use evaluations, quality improvement initiatives, and committees
- Provides mentorship and supervision to pharmacy students, and new staff.
- Additionally, we would depend on your expertise to ensure that prescriptions are validated and dispensed accurately; and that medication safety concerns, working with the health care team to resolve discrepancies and/or problems are resolved expediently; your professional practice activities shall be documented proficiently.

- Experience working with word processing, spreadsheet and database applications.

- Demonstrated pharmacologic knowledge (e.g. pharmacodynamics and pharmacokinetics).
- Ability to plan, implement, organize, problem solve, and work on several projects simultaneously.
- Self-motivated, reliable, and patient focused team player.
- Good attendance and availability required.
