Thank you for your application to be a preceptor with the Leslie Dan Faculty of Pharmacy. Please note that at this point we have concluded our preceptor application process for community pharmacy sites for this experiential year. We will be reassessing our program needs on a yearly basis, and will be contacting you about next steps at the end of this academic year. If you are in a practice that offers unique experiences to our students, such as administration of flu vaccines or other injections, smoking cessation service/clinic, warfarin monitoring, methadone clinics or other unique types of services/clinic, or are located in any of the following geographic areas: London, Windsor, Kingston or Ottawa, please feel free to send us an email at<>.

Library Cards for APPE Preceptors

APPE Preceptors will have access to University of Toronto Libraries only during the APPE academic year in which they precept an APPE student. Please email to request a library card applicatiuon form.
